This is the only hot pack that works and stays hot for a long time, and I bought several items for it this year, but it was back to normal when it started to cool down.
I haven't used it as a Cold Pack yet, but I have used it as a heat pack, and here are the pros and cons of each one, as well as the drawbacks of the gel packs that I just purchased.
It can be used on top of your back or on your neck, it's meant to heal and believe me, it will.
I have had back pain for the past 15 years from a few bulging discs and I don't know what I would do with the moist heat.

I've used these buggers for more than a decade and have never had a problem with them, although I've probably bought fewer than 4 of them.
I put it in the freezer for 3 hours and it doesn't get cold. What time does it need to be in the freezer?
I have 6 of these, 2 are cold packs and 4 are hot packs. For the Hot Packs I just take them as-is and nuke two-at-a-time for 1:35mins on high in a 1100 watt microwave oven.
I received a product that had no instructions, can someone tell me how to microwave, etc?
I microwave it for a minute and it works for me.

It's microwavable, just make sure it's not too hot before applying.
